Adding Forms to Your Storefront
Section titled “Adding Forms to Your Storefront”FormFlow provides two ways to get your forms onto your storefront:
- App blocks — embed a form directly on a Shopify page using the theme editor (requires an Online Store 2.0 theme)
- Embed code — paste an iframe snippet into any theme template manually

Requirements
Section titled “Requirements”App blocks require a Shopify theme that supports Online Store 2.0 (OS2). Most modern Shopify themes do. If your theme isn’t compatible, FormFlow will show a notice and you’ll need to use the embed code method instead.
Adding a Form to a Page
Section titled “Adding a Form to a Page”- Open the form in the Form Builder
- Click the Usage tab
- Click Add to Page
- Choose the page type: Pages, Blog Posts, or Products
- Search for and select the specific page
- Choose a position: Bottom of template (default) or Top of template
- Click Add
FormFlow adds the form as an app block to that page in your theme. The form appears on your storefront immediately.
Viewing and Removing Placements
Section titled “Viewing and Removing Placements”The Usage tab shows all the pages where a form is currently embedded:

| Position | Where the form appears on the page (top or bottom) |
To remove a form from a page, click Remove next to that placement. The form block is removed from the page immediately.
Using the Embed Code Instead
Section titled “Using the Embed Code Instead”If you prefer to control placement manually or your theme doesn’t support app blocks, use the embed code snippet from the Embed tab in the Form Builder. See Public Forms for details.
